Medical Forms

This is your weekly reminder that a full medical form is required for summer camp. Please note that Michelle Mitchell & Jen Oberst are collecting them, so contact them with details if you have not submitted it yet – mitch6633@aol.com / 951-275-7939 or jkoberst@gmail.com / 509-280-8736. We’d like them earlier, if possible.

Please verify all the following:

• BSA Medical form – complete with A, B & C
• Verify that “List any Participants restrictions” has the NONE box clicked or your son may not be able to participate in certain activities
• Part C must be signed by a doctor no later than 07/08/2018 (must be within a year of the start of camp)
• Copy of your insurance card, front and back
• Over the Counter medical form
